To prepare for your visit to the salon you may read and complete the appropriate electronic documents and forms prior to your visit. If you do not complete the forms in advance, please give yourself 10-15 minutes before your appointment to fill out the paperwork in the salon.

Complete forms will be emailed to us upon completion.

Documents for informational purposes regarding your appointments may be downloaded and saved if you would like to retain a copy.
